Important Note for Existing ChARM Users

If you've used ChARM PHR with another provider before:

  1. Log out completely from your existing account

  2. Then click the activation link to connect our clinic to your account

Step 2: Complete Required Intake Forms

⚠️ Must be completed before your scheduled visit

 

How to Access Forms

  1. Log into your ChARM patient portal

  2. Click on the "Questionnaire" tab

  3. Complete all required forms

Step 3: Submit Previous Lab Results

If You Have Access to Lab Results

  • What to include: Lab results from the past year

  • How to submit:

    1. Upload files to your ChARM portal under "Documents"

    2. Important: Click "Share" after uploading

    3. PLEASE MAKE SURE ALL DOCUMENTS ARE PDF FORM (Excel, Microsoft Work, Pages etc are not readable when accessing through charm.

Request your previous/current healthcare providers to fax results directly to us:
